During the weeks leading up to Diane Keaton's death, her close friends noticed alarming changes in her health. Her longtime friend, Grammy and Oscar-winning songwriter, Carole Bayer Sager told PEOPLE that she was 'stunned' when she saw the actress weeks before her death. Carole and Diane had shared a deep friendship for decades and even collaborated on the actress's solo single, First Christmas released in late 2024. Despite her frail appearance, Carole mentioned that Diane remained her 'vibrant and creative self' like always. In her words:

"I saw her two or three weeks ago, and she was very thin. She had lost so much weight. She had to go to Palm Springs because her house had been damaged inside, and they had to clean everything. She was down there for a while, and when she came back, I was kind of stunned by how much weight she'd lost."

Another close friend echoed Carole's concern and told the outlet that the actress's health had "declined very suddenly." The friend revealed that in her final months, the actress had become more private and limited in her social life than ever. Despite being reserved, her funny personality turned every simple moment into something special. Another source claimed that Diane was surprisingly not seen walking her dog, Reggie, around her Brentwood neighborhood in recent months, a routine that had once been a daily occurrence for her.

What happened inside the final 'dispatch call' for Diane Keaton's home? 

According to TMZ, the final dispatch call from Diane Keaton's home was made on the early morning of October 11, 2025. The call log noted, "Rescue 19, person down." First responders had immediately arrived at her property at 8:08 a.m. ET. She was quickly transported to a nearby hospital, where she was later pronounced dead. Although no official cause behind her death was disclosed in the long run, sources told US Weekly that Diane was dealing with undisclosed health challenges in the months before her death. However, she chose to keep them private.

Diane Keaton looked healthy in her final Instagram post 

Despite Diane Keaton's declining health, she appeared cheerful in her last Instagram post shared in April 2025. The post was shared in collaboration with retailer, Hudson Grace, where she was seen cuddling her dog to celebrate National Pet Day. The actress was seen dressed in her signature neutral tones and hat, where the Father of the Bride star flashed her iconic smile. It has now become a bittersweet moment for fans. The caption read: 

"Proof our pets have great taste too! Happy National Pet Day from HG & @diane_keaton."
