The second episode of Doc Season 2, titled Delusions of Grandeur brings back the high-stakes drama at Westside Hospital. The show combines intense medical cases with personal challenges and a doctor who has no recollection of the past eight years of her life. The episode also introduces 'Dr Joan Ridley' (played by Felicity Huffman), who stirs up unresolved tension between 'Amy' and 'Michael' while they attempt to navigate complex medical situations.
The episode begins with a young woman named 'Megan' who believes that she is pregnant despite no evidence of a fetus. The patient lives in a commune that rejects modern medicine. 'Megan' presented with pregnancy-like symptoms such as nausea and abdominal pain. However, after performing an ultrasound, 'Dr Amy Larsen' discovers that it's not a fetus but a giant mass. Further tests confirm that Megan isn't pregnant but is suffering from pseudocyesis. For the unversed, it's a psychological condition where the body mimics pregnancy symptoms.
'Amy' and 'Gina' further decide to play along with 'Megan's' beliefs to avoid distress. However, complications arise when a woman from 'Megan's' commune, 'Ann' interferes and prevents her from undergoing medical tests. Eventually, 'Amy' and 'Gina' convince 'Megan' to undergo an MRI, which reveals a tumour in her abdomen. After some emotional turmoil, 'Megan' agreed to a biopsy, and the tumour was successfully removed. To add to 'Megan's' surprises, 'Gina' helps her reconnect with her long-lost brother, 'Jordan', who provides comfort and support as she comes to terms with the truth about her condition.
Another surprise for Doc Season 2 fans was the return of 'Dr Joan Ridley' after a five-month absence. It marked a significant development in the episode as she was also 'Dr Amy's' former mentor, who returns to claim her position as Chief of Internal Medicine. Her return raises tension for 'Amy', who is back to being an intern. However, while 'Joan's' presence reignites old memories, one of which involves a dinner plan that led to 'Amy' rejecting 'Michael's' attempt at reconciliation.
'Dr Joan's' medical skills are proven when she successfully diagnoses a patient with persistent hiccups caused by a lipoma. However, her influence on 'Amy' takes centre stage as it is revealed that she played a part in encouraging 'Amy' to break up with 'Michael' years ago, adding layers to their strained relationship.

In the middle of everything, 'Michael' offers 'Dr Joan' the role of Chief of Internal Medicine, which she happily accepts. However, this decision only adds to the tension between him and 'Amy'. As 'Gina' informs 'Amy' about the promotion, it triggers a series of flashbacks for the doctor. Nonetheless, it all starts with a chocolate cake, which eventually leads to the memory of 'Michael's' attempt to mend his relationship with 'Amy' by inviting her to a concert, but she chose to dine with 'Joan' instead, leading to the eventual breakup.
